New Patient Registration

All persons making an application to join our practice list must do so by requesting an application pack from our reception staff or by downloading the required forms below.

Please check with reception staff that you live within our practice boundary - a map detailing practice boundaries is available at reception.

Completed forms should be returned to reception between 2pm and 5pm to facilitate registration.

Should registration not be accepted the practice will provide a reason in writing within 10 working days.

An appointment for a new patient check-up will be made with either a nurse or health care assistant for all newly registered patients. This is done to ensure that the practice offers the most appropriate medical service to each of its patients.

Patients may make an appointment with their doctor prior to a new patient check appointment.

Summary of Registration Procedure:

  1. Pick up and complete a registration pack from reception or you can download the forms from above
  2. Complete a GMS1 form (Family Doctor registration form)
  3. Bring both the completed registration pack and GMS1 form along with 2 forms of identification (One must photographic ID) to the surgery between the hours of 2pm and 5pm
  4. A new patient check-up will be made with either a nurse or healthcare assistant
